ESCAPE CODE PHOTO/AUDIO/VIDEO RELEASE FORM

I hereby grant Escape Code Branson, LLC permission to use my likeness in a photograph, video, or other digital media (“photo”) in any and all of its publications, including web-based publications, without payment or other consideration.

I understand and agree that all photos will become the property of Escape Code Branson, LLC and will not be returned.

I hereby irrevocably authorize Escape Code Branson, LLC to edit, alter, copy, exhibit, publish, or distribute these photos for any lawful purpose. In addition, I waive any right to inspect or approve the finished product wherein my likeness appears. Additionally, I waive any right to royalties or other compensation arising or related to the use of the photo.

I hereby hold harmless, release, and forever discharge Escape Code Branson, LLC from all claims, demands, and causes of action which I, my heirs, representatives, executors, administrators, or any other persons acting on my behalf or on behalf of my estate have or may have by reason of this authorization.

• Photos: Photos and/or video will be taken and included in our marketing materials (including, but not limited to, our website, Facebook page, fliers, television commercials and advertisements). By signing this agreement you give Escape Code Branson, LLC permission to use photographs and videos which may include your image and video footage without restriction or compensation.
• I understand there is audio and video monitoring and recording in every room, and consent that it is okay.

I have read this Agreement and understand that by signing the Agreement I have consented to be bound by its terms, including the waiver/release of any legal right I may have to sue Escape Code Branson, LLC for any costs they incur because a claim or legal action is brought in violation of this Agreement.
